Our Case Management program includes a complete assessment for all clients entering Casa Youth Shelter. We develop an appropriate and individualized Case Management Service Plan with clients and/or their families as the means for implementing appropriate referrals and developing effective alliances with clients and any existing treatment teams. We provide on-going supportive and case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients achieve their stated goals and objectives. This includes implementing face-to-face reviews with the client, families and/or clinical staff on a regular basis so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. We participate in inter-agency planning and service coordination activities as necessary to ensure a smooth transition into an appropriate aftercare setting. Our Case Manager provides post-therapeutic services and referrals, as well as conducts periodic follow-ups with the clients and their families, offering additional resources as needs change.
